What is a CBRF?

A Community-Based Residential Facility (CBRF) is a licensed group living setting where five or more unrelated adults live together and receive care, supervision, and support. CBRFs provide assistance with daily living activities, medication management, and social engagement, while promoting independence and dignity. (Reference: Wis. Admin. Code DHS 83.01)

How is a CBRF different from a nursing home or RCAC?

A CBRF offers 24-hour supervision and assistance with daily living but not skilled nursing care. An RCAC (Residential Care Apartment Complex) provides more independence with up to 28 hours of supportive services weekly. A nursing home provides 24-hour skilled medical care for higher medical needs.

Who regulates CBRFs in Wisconsin?

CBRFs are licensed and regulated by the Wisconsin Department of Health Services (DHS), Division of Quality Assurance (DQA). DHS conducts regular inspections to ensure compliance with Wis. Admin. Code DHS 83.

What services are provided in a CBRF?

Services may include assistance with daily living (bathing, dressing, grooming, toileting), medication management, meals, laundry, 24-hour supervision, memory care support, recreational activities, and coordination of medical appointments.

Who is eligible to live in a CBRF?

Adults (age 18+) who need help with daily activities or supervision but do not require continuous skilled nursing care. A pre-admission assessment ensures the facility can meet their needs safely. (Reference: DHS 83.07)

How are care needs assessed?

Each resident receives a comprehensive assessment before admission and at least annually, identifying support needs, preferences, and abilities. This forms the basis of the Individual Service Plan (ISP). (Reference: DHS 83.35)

What is an Individual Service Plan (ISP)?

The ISP outlines the services, supports, and goals tailored to each resident, developed collaboratively with the resident, guardian (if applicable), and staff.

Is there a nurse staffed 24 hours/day?

 There is a nurse in the building daily, Monday-Friday and on call after hours and on weekends.  The Evergreens will have Resident Care Assistant staff in the building 24/7

Can residents have visitors?

Yes. Residents can welcome visitors at reasonable times, consistent with their preferences and household routines. Family involvement is encouraged while ensuring safety and privacy. (Reference: DHS 83.32(3)(h))

Can residents bring their own furniture and belongings?

Yes. Residents are encouraged to bring familiar furnishings and personal items, as long as safety and accessibility standards are met.

How is medication managed?

Depending on ability and facility policy, staff may administer medications or supervise self-administration. All medications are securely stored and documented per DHS 83.37.

What are the staff qualifications?

All staff receive DHS-mandated training, background checks, CPR/first aid certification, and at least 12 hours of continuing education annually. (Reference: DHS 83.25)

How are meals handled?

Residents receive three balanced meals and snacks daily. Special diets such as diabetic or low-sodium can be accommodated based on dietary needs.

What are the resident rights in a CBRF?

Residents have the right to privacy, dignity, participation in care decisions, financial autonomy, and freedom from abuse or neglect. (Reference: DHS 83.32)

What happens if a resident’s needs change?

Staff reassess the resident and update the ISP. If needs exceed what the CBRF can provide, staff assist with a safe transition to another care setting.

How much does it cost to live in a CBRF?

The Evergreens will be charging an all-inclusive rate of $225/day.  This includes room and board, meals, assistance with ADL’s per the resident ISP, and medication management/administration.  Some residents pay privately, while others qualify for public funding through Family Care or IRIS programs.

Is financial assistance available?

Yes. Wisconsin’s Family Care and IRIS programs can help eligible residents with costs. Contact your local Aging and Disability Resource Center (ADRC) for eligibility information.

What safety measures are in place?

CBRFs maintain fire safety systems, conduct regular drills, and have trained staff on-site 24/7. Infection control and emergency procedures are in place. (Reference: DHS 83.47)

Can residents have pets?

No-The Evergreens cannot accept pets.  We do welcome visiting pets with proper vaccination records.

What is resident-centered (or individualized) care?

This approach ensures that residents’ routines, preferences, and goals shape their care and environment. Staff work collaboratively to support independence and dignity.
