When filling out a CRB Report, please provide as much information as possible. Anonymous reports are accepted, but will limit the team’s ability to offer support.

All complaints will be received by CRB members, and if a response is requested, follow-up will occur within 72 hours.

This online form is just one of several ways that you may share a concern. Concerns can also be shared by emailing [email protected] or calling 608-785-5796.

The CRB meets monthly to discuss reports, document patterns, and determine what information needs to be shared with the community and/or law enforcement.

• This report form is NOT intended for an emergency response. If you need to report an EMERGENCY or incident that poses an imminent risk of harm, or if there is an immediate risk or threat to safety, security, or health, CALL 9-1-1 IMMEDIATELY.
• All reports will be handled with sensitivity.
• Because the CRB is not an investigatory body, we receive all reports as shared experiences without needing to prove or validate them, allowing us to focus on providing support rather than substantiating claims.
• The CRB values honesty and expects all reports of police conduct to be made in good faith.
