WE ARE OPEN TO THE PUBLIC FOR SAME DAY OVER THE COUNTER SERVICE.  

Vital Records applications will be accepted in-person Monday thru Friday (except holidays) from 8:00am until 4:15pm. Counter customers applying after 4:00pm will have their applications processed next day.

Birth, Death, Marriage & Divorce Information and Applications

We issue certified & uncertified copies of birth, marriage, death & divorce certificates for those records where the event occurred in Wisconsin. If the event occurred outside of Wisconsin, your certificate will not be found here. We can issue birth certificates for ALL births that occurred in Wisconsin from 1907 to present. We can issue ALL marriage certificate for ALL marriages that occurred in Wisconsin from 1907 through present. We can issue any death certificate for all Wisconsin counties from September 1, 2013 to present. We can issue any divorce certificate from January 01, 2016 to present.  If the death record you need occurred prior to the date above, you would need to contact the county where the death occurred. If the divorce happened in La Crosse County but prior to 1-1-2016, you would need to contact the Clerk of Circuit Courts office or State Vitals in Madison. Wisconsin Counties         States

YOU MUST HAVE A DIRECT AND TANGIBLE INTEREST IN THE CERTIFICATE YOU ARE REQUESTING. Generally, this means the immediate family only, i.e., spouse, parent, child, brother, sister or grandparent.

If you are able to come into the office, our hours are 8:00 a.m. to 4:15 p.m. Monday through Friday. You will receive over-the-counter, same day service. Birth, Death, Marriage, & Divorce records are $20.00 for the first copy, and each additional copy purchased at that same time of the same record is $3.00. There is no limit to quantities. Identification is required. Acceptable Forms of ID.  We accept cash, money orders, certified checks, or credit & debit cards (there is an additional fee for credit/debit).

If requesting your certificate by mail, please use the correct application provided herewith. Fill in the applicant information (including signature), relationship, and the number of copies. One form will be needed for each individual record you are requesting. We require a money order or a cashier's check for all mail requests. Make these payable to Register of Deeds. DO NOT send a personal check. It will be returned to you for replacement. Please include a self-addressed, stamped envelope ** & you must include a photo copy of a valid ID such as a driver's license, state ID card OR 2 from the list given on the back of the application. All requests that meet the required criteria will be processed and mailed the same day the request is received. **If you would like your certificate(s) mailed back by certified mail, please add an additional $6.50 to your money order or cashier's check, you would then not need the SASE.

**IMPORTANT: Identification and a signature for the applicant are required for all vital records requests ordered online or through the mail**

(If you do not have a valid photo ID then you will need to submit 2 of the following forms: Bank statement, Current dated signed lease, health insurance card, utility or traffic bill, vehicle registration/title)
