Transportation Contracts - Nancy Schmidt and Chris Sander |

Nancy and Chris presented a handout and information on transportation options and funding methods. The handout includes information on ambulance, common carrier, special medical vehicle, nursing home van, volunteer drivers, friends, public transport, mini bus, taxi and supportive home care. The transportation budget in La Crosse County grew 3 times since implementation of the MCO. There are different rates according to services and location. Currently, one person in La Crosse county spends 80% of her time on transportation.
The group discussed a regional service for transportation with a coordination of services. The discussion included the option of utilizing a joint RFP. It was felt that there may be purchasing power if all the counties would go together on an RFP. Another option would be for the MCO to go out on bids on its own. It was recommended that transportation be brought back to the group for further discussion. A subcommittee will be established with individuals from each county that handle transportation services to further explore joint contracting options. |

Case Record Transfers |

Maryellen presented recommendations on information that should be included in case record transfers. The group discussed whether the whole file should be transferred to the MCO or only items identified on the handout. If the whole file isnt transferred, who will be responsible for making copies? The MCO likely will keep all current La Crosse CMO records since those records are already housed in the current CMO and future MCO site; however, taking all case files from all counties would create storage and potentially liability problems for the MCO. The MCO would like the case records 2 weeks prior to enrollment in each county. Making copies of the charts may create problems for some counties due to workload issues. One suggestion was to use a temp to scan records onto electronic media. The MCO may also be able to send staff to assist; however, it may be easier for caseworkers to find the requested records for their clients.
Another suggestion was to contact other counties who have already faced this situation and see how they handled this situation. Phil Wicktor called Chippewa County Human Services Director Dave Rynders, who reported his county copied 12 months of case notes and brought those with assessments and other current documents to CHP but kept the actual client files. Dave noted that Dunn handed over only six months worth of documentation. The West Central Wisconsin consortium will discuss whether to make the amount and type of information given to CHP consistent across all counties in that service area.
A question arose regarding court records for protective placements. Each county was to go back and discuss this issue with their Corp. Counsel and bring recommendations to the next meeting.
An MOU needs to be developed between the Western Wisconsin Cares MCO and the partner counties. This will be discussed further at the next Steering meeting. |

MCO/County MOUs - IMD Discharge Plans - Disaster Planning |

Draft MOU language was distributed with the understanding that several addendums need to be developed. The MCO contract requires two specific MOUs one for disaster planning and another for IMD discharge planning. Since the proposed language on disaster planning is short, that section is included within the main body of the MOU. A recommendation was made to include Health departments, as well as Emergency Management, in the language. The IMD language is included as a separate addendum. The MCO will work with county case managers on how to plan for community placement of individuals who are discharged from IMDs and were or could be MCO members. Much of the draft language was based on MOUs approved by the State in other counties.
Other addendums will be drafted and brought back to Steering to address Adult Family Home certification, crisis services and other areas where collaboration will be important. For HIPAA purposes, it will be important to draft a Business Associate Agreement as well. |

Personnel Update |

Bev has requested information from county personnel directors regarding staff who may be interested in transferring the MCO. Gathering information on employee claims will be important to get insurance quotes. In addition, the group discussed the financial implications for the transfer of vacation, sick and comp time when individuals transfer from a county to the MCO. A discussion was held regarding the possibility of the county and MCO splitting liability for these benefits because employees are more likely to accept work with the MCO if they dont lose anything by leaving the county. The MCO also is seeking legal counsel regarding the actual hiring process to ensure all state and federal regulations are met. |

ADRC Updates

The ADRC of Buffalo, Clark and Pepin Counties is ready to send in its application to the State. The Aging departments will take the lead in that group.
The ADRC of Western Wisconsin is still awaiting state approval of the proposed enrollment plan; however, those counties are moving forward with regionalization. Jackson and La Crosse counties will merge their ADRC operations on July 1. The four-county advisory board is almost ready to start meeting, as 10 of the 11 members are in place. |
