Admissions...the smooth transition.

Hillview offers admission to anyone requiring 24-hour nursing care, either on a temporary or long-term basis.  The majority of our admissions are short term.  50% of all discharges return to their previous environment.  Our social workers will work with the family, outside agencies, hospitals or other health care providers to assure that residents experience smooth transitions.

All of our social workers are Certified Advance Directive Counselors, Power of Attorney for Health Care and are adept at handling, admissions, financial assistance, general transfers or discharges, advance directives, family issues, Medicare/Medicaid, and resident personal phone service.
